Eczema Not Responding to Steroid Cream? There May Be a Better Option

You have had eczema for years. You know the drill. It flares up, you put the cream on, it calms down, you stop, and a few days later it is back. Except recently it seems like the cream is not working as well as it used to. The flares are more frequent. They are spreading to new areas. And when you try to stop using the cream altogether, your skin goes haywire.

If any of that sounds familiar, you are not imagining it. And you are definitely not alone.

The steroid cream cycle

Topical steroid creams are one of the most commonly prescribed treatments for eczema in the UK. For most people, used as directed and for short periods, they work well. They reduce inflammation, calm the itch, and let the skin settle down.

The problem comes with long-term use. When steroid creams are applied regularly over months or years, the skin can start to depend on them. It adapts to having the steroid present and stops being able to manage inflammation on its own. When you then try to reduce the cream or stop it altogether, the inflammation rebounds, often worse than it was before.

This is not your eczema getting worse. It is your skin reacting to the absence of the steroid. It is a recognised condition called topical steroid withdrawal, or TSW.

 

Signs you might be in the steroid cycle

  • Your eczema keeps coming back within days of stopping the cream
  • The cream that used to work does not seem to be effective any more
  • You have been moved to a stronger steroid and are worried about what comes next
  • The redness and irritation is spreading to areas you never had eczema before
  • Your skin burns or stings when you apply the cream, rather than feeling soothed
  • You feel anxious about what will happen if you stop using it

These are all signs that your skin may have developed a dependency on the topical steroid. It does not mean you did anything wrong. It simply means that the treatment has run its course and your skin needs a different approach.

 

What are the alternatives?

The good news is that there are options beyond the steroid cycle. Depending on your situation, these might include:

  • Gradual reduction of steroid use under clinical guidance, sometimes called a weaning programme
  • Intensive moisturising and barrier repair to support the skin during withdrawal
  • Identifying and avoiding triggers that worsen your eczema
  • Cold plasma therapy, which is an emerging treatment that reduces inflammation, fights bacteria on the skin, and supports natural healing without any chemicals or creams

Cold plasma is particularly interesting for people caught in the steroid cycle because it does not add anything to the skin. For skin that has become sensitive to everything, that matters.
